Annual Update (Emergency Card)
Emergency cards and other important information are now processed online! Please sign in on portal.pvschools.net, select "More" and then "Annual Update". Here you will be able to update contact information with any address changes, student health history, and emergency contact information. These updates are required every school year!

Find your home school using our school locator tool. If you require assistance in finding your home school, please call us at (602) 449-2000 and ask for Elementary or Secondary (middle and high school) Support.
If Shea Middle School IS Your Home School:
Use this link to register a new student, then fill out this paperwork to accompany the online form. At the time of submitting your registration paperwork, we will also require a copy of your current proof of address, your child’s Birth Certificate, and Immunization Records. Acceptable forms of proof of address are:
Valid Arizona Driver’s License
Rental agreement or lease
Utility Bill (water, gas, cable)
Bank or credit card statement
Valid Arizona motor vehicle registration
New students will also want to complete this class registration form for Shea so that they can request their electives for the year.
If Shea Middle School Is NOT Your Home School:
You can still apply to join our community! The first step is to visit the Open Enrollment page to view information about our schools. Please note that Open Enrollment applications for the 2021-2022 school year will only be accepted online and paper applications will no longer be accepted.
If your child has an IEP, please include a copy of his/her most recent IEP documents for our school Psychologist, Jenn Whitted to review. If you have any questions about this process, you can email her at firstname.lastname@example.org.

You can pay for school meals (breakfast, lunch and snacks) online using the student's ID number and the zip code of the school (85028). Breakfast is $1.25, lunch is $2.75, and snacks range from .25 to .75 (limit 2 snacks per day). Free and reduced applications can be filled out online and must be filled out every year (one application per household).
